If you are having a life threatening emergency, call 911 or report to your closest emergency room.

If you are signed up with MyChart, you may cancel your appointment online or through the mobile app.

If you don’t have MyChart, please call (919) 537-3737 and select:

• Option 3 for a faculty provider
• Option 4 for a resident provider
• Option 5 for a student provider

Please note, patients who do not show up for appointments may be charged a cancellation fee.

Patients who repeatedly break or cancel appointments without at least 48 hours notice may be dismissed from Carolina Dentistry at the discretion of the dental provider managing the patient’s care.

Patients should park in Dogwood Parking Deck. An approximate GPS location for Dogwood Parking deck is 170 Manning Dr., Chapel Hill, NC 27599.

Carolina Dentistry patients park for free during their appointment. Please be sure to get a voucher from the front desk during check out.

If you have mobility challenges, please park in the Ambulatory Care (ACC) lot and take the free UNC Health shuttle. Be sure to tell the driver you have an appointment with the dental school.

Payment methods and times of payment vary by provider level.

Faculty providers accept MetLife dental insurance. Resident and student providers accept Medicaid.

Patients may self-pay (Visa, MasterCard, American Express, cash or check) with any of our providers.

You are encouraged to discuss payment options and questions with Patient Business Services. Call (919) 537-3737 and select option 2.

If you feel that you have not received fair treatment you may report your concerns via MyChart or by sending an email to carolinadentistry@unc.edu.

Your Rights as a Patient

Treating the Person: Carolina Dentistry recognizes and respects the dignity of each patient. You should expect to be treated with consideration and respect regardless of age, color, disability, gender, gender expression, gender identity, genetic information, national origin, race, religion, sexual orientation, veteran status, or source of payment.

Appropriate Services: Carolina Dentistry will provide services consistent with the patient’s needs. You may be informed about what can and cannot be provided, and your providers will make referrals for treatment when necessary. When your relationship with Carolina Dentistry ends, no matter the reason, you will be informed of remaining treatment needs.

Understanding the Plan of Care: Carolina Dentistry patients are entitled to a clear explanation of their dental problems, the recommended treatment(s), the anticipated results of treatment, the risks involved, and any alternative treatment options. You may receive an estimate of the costs and how long treatment may take. When complications come up during treatment that might change the plan of care or affect the anticipated results, you will receive a full explanation.

Consent and Refusal of Treatment: Carolina Dentistry patients have the right to participate in decisions about their dental treatment and have questions answered before deciding. You may refuse treatment and should expect to be fully informed of the possible risks of foregoing treatment.

Confidentiality: Patient privacy rights are protected under the Health Insurance Portability and Accountability Act (HIPAA), applicable state laws, and Carolina Dentistry policies. All faculty, staff, residents, and students are required to abide by these laws and policies. Your information will not be disclosed without your written permission, except as permitted by law and stated in the Carolina Dentistry Notice of Privacy Practices.

Your Responsibilities as a Patient

As a patient or caregiver of a patient in our clinics, your responsibilities are to:

• Be considerate and respectful of other patients, learners, and employees.
• Share honest and complete information about medical and dental history, previous illnesses, hospitalizations, exposure to communicable diseases, allergies, medications, and current medical care.
• Let the provider know when there are changes in the patient’s general health condition, and when patients experience unusual discomfort or complications following a treatment.
• Ask questions and understand the nature of dental conditions and treatments.
• Follow the instructions given to follow up on treatment.
• Be available to make appointments during the entire treatment phase; to keep scheduled appointments; and to arrive for appointments on time.
• Consider that the school strongly encourages patients to have all dental treatment done in the student clinic to which you are admitted, except in case of emergency or when referred for specific treatments by an authorized Adams School of Dentistry provider.
• Pay for all services when received unless the Carolina Dentistry has approved other arrangements.
• Update all address and phone number changes directly with the main desk of the appropriate clinic as soon as possible.

Not meeting these responsibilities can lead to patient dismissal.

Sus Responsabilidades Como Paciente

Como paciente o cuidador de un paciente en nuestras clínicas, sus responsabilidades son las siguientes:

• Ser considerado y respetuoso con otros pacientes, alumnos y empleados.
• Compartir información honesta y completa sobre historial médico y dental, enfermedades previas, hospitalizaciones, exposición a enfermedades transmisibles, alergias, medicamentos y atención médica actual.
• Informar al proveedor cuando hay cambios en el estado de salud general del paciente y cuando los pacientes experimentan molestias o complicaciones inusuales después de un tratamiento.
• Hacer preguntas y entender la naturaleza de las condiciones y tratamientos dentales.
• Seguir las instrucciones dadas para realizar el seguimiento del tratamiento.
• Estar disponible para programar citas durante toda la fase de tratamiento; cumplir con las citas programadas; y llegar a tiempo a las citas.
